Medication Update:
Medication will not be administered without MD and parental authorization. This includes over the counter products (Tylenol). Medication permission forms are available in the health office and on the school website. All medication (including over the counter products) must be submitted in the original container. Medication must be delivered to the health office by the parent/legal guardian

Physical Examination (forms available in main office)
(May be performed by private MD or school physician)
Completion of Immunizations:
1. 5 doses DPT/DTaP
2. 4 doses OPV
3. 2 MMR
4. 3 Hep B
5. Varicella (by 9/04)
