The New Jersey Department of Health and Senior Services (DHSS) has recently revised the administrative rules with substantive changes to include the requirement of four new vaccines for school, preschool and licensed child-care center attendance in september 2008. These vaccinations will include DTP/DTap, Pneumococcal conjugate, Influenza and Meningococcal vaccines. For details, please refer to the February 2008 DHSS health release.

First Grade Registration

You will find pertinent registration forms below for registering a child who is currently attending a private kindergarten and will be entering 1st grade for the 2008-09 school year. Please return these completed forms to the Upper Freehold Regional Elementary School Main Office of the Upper Freehold Regional Elementary School beginning March 31, 2008 and ending April 30, 2008. The forms appear in pdf format for ease of download.
