Medication Update:
Medication will not be administered without MD and parental authorization. This includes over the counter products (Tylenol). Medication permission forms are available in the health office and on the school website. All medication (including over the counter products) must be submitted in the original container. Medication must be delivered to the health office by the parent/legal guardian

(must be completed and returned by the first week of school, please include cell number)

Grades 5, 6, & 8

Sport Physical Exam**
** Every student who is participating in a sport must have a sport physical every 365 days.
Medical updates are performed by the nurse for each subsequent sport during the 365 day period.
Sport permission forms and other paperwork must be submitted for each sport.
Available by the school physician by appointment for sports only.
All paperwork and physicals should be completed and submitted prior to the first day of practice.
Every student entering the district must have completed a physical examination within the past 365 days and have met all immunization requirements.
